Job Summary: The Community Specialist (CS) supports the VNA Connections, Volunteer, and Bereavement programs. Working under the guidance of each program leader, the CS provides administrative and program support, including client contact, event planning and coordination, and clerical assistance as needed.
The CS collaborates regularly with internal teams, initiates case conferences when appropriate, and delivers presentations to support program awareness. The role also assists with education and outreach within VNA and the broader community to promote understanding and effective use of VNA programs, and may help train new hires on job responsibilities and processes.

Responsibilities:
- Collaborate with clients, families, and caregivers to implement and support individualized plans of care.
- Monitor client needs, identify changes, and coordinate with medical providers and community resources to ensure appropriate services and continuity of care.
- Educate clients on available resources, provider instructions, and assist with referrals, applications, and care navigation.
- Conduct follow-ups and bereavement calls to assess progress, support families, and promote quality of life.
- Build and maintain relationships with healthcare providers, community partners, and donor organizations.
- Support volunteer programs and assist with planning and executing events.
- Maintain accurate client documentation, communicate program benefits, provide office support when needed, and travel within the service area.
Education/Experience/Skills/
Qualifications:
- High School diploma or GED.
- Background/education in Social Services or Community Activism.
- One (1) year of experience working in a healthcare setting, preferably with a home health or hospice agency.
- Strong interpersonal and communication skills.
- Proficient in the Microsoft Office Suite.
- Bilingual in Spanish strongly preferred.
