Job Summary

The Early Start Service Coordinator at San Andreas Regional Center is responsible for coordinating and facilitating services for infants and toddlers with developmental delays and disabilities. This role involves working closely with families, service providers, and multidisciplinary teams to ensure that children receive the necessary early intervention services and support to reach their full potential. The Service Coordinator plays a key role in improving the lives of children and their families through assessment, planning, and ongoing support.

Job Functions
  • Collaborates in psychosocial and developmental screening assessments in the home environment.
  • Demonstrates a strong commitment to serving the community by being willing to travel extensively within assigned catchment areas that SARC covers.
  • Conducts vital home visits in diverse settings, with emphasis on the child and family’s natural environment to tailor support and interventions.
  • Develops Individualized Family Service Plans (IFSP) in collaboration with families and other professionals.
  • Facilitates communication and coordination among service providers, therapists, and specialists to ensure the child’s development and progress.
  • Develops, implements, and coordinates an IFSP for each individual.
  • Coordinates services for infants and their families; determines ongoing eligibility of clients at age 3.
  • Completes required forms, documentation, and reports according to regulations and SARC policies.
  • Promotes advocacy by parents and helps them become their children’s teachers in everyday life skills, providing informational support to families and ensuring active engagement in decision-making.
  • Schedules and facilitates annual and semi-annual reviews, transition planning conferences, clinical consultations, and /IFSP Team meetings as required.
  • Works collaboratively with service providers and community partners to monitor a child’s progress toward IFSP goals.
  • Maintains accurate records of assessments and IFSPs, ensuring regulatory compliance.
  • Supports the child’s transition from Early Start to educational services available after age 3.
  • Provides advocacy, education, crisis intervention, and other support for families and care homes.
  • Transfers, synthesizes, and reports individual statistical data as requested (state surveys, internal surveys, community agency surveys).
  • Participates in internal and external committees to promote public education, improve agency functions, and foster positive communication with external agencies.
  • Engages in in-service and out-service training to increase knowledge and skills in developmental services.
  • Stays up to date with current early intervention practices and regulations through ongoing professional development.
  • Serves as Officer of the Day (OD) as assigned.
  • Completes required non-individual related documentation, including travel input, time logs, and monthly activity forms.
  • Gathers data and produces initial and ongoing assessments; presents assessment data to the Interdisciplinary Team (IDT) for eligibility determination.
  • Facilitates planning and develops IFSPs for assigned individuals; monitors and evaluates services provided through the plan.
  • Advocates with individuals and families to assure provision of needed services; initiates and follows up on referrals to community resources.
  • Identifies areas of unmet needs and performs tasks related to fiscal and accountability requirements.
