Nurse Case Manager – Gilroy and Salinas, CA

• Administer medical case management for work‑related injuries, including assessment, planning, coordination, implementation, and evaluation of injured or disabled individuals.
• Collaborate with insurance carriers, medical care providers, attorneys, employers, and employees to monitor progress and report results to stakeholders.
• Perform home‑based visits in the Gilroy and Salinas areas, traveling up to 2 hours one‑way (4 hours round‑trip) and attending multiple appointments per day as needed.

• Current, unencumbered Registered Nurse (RN) license in California.
• Experience in case management of workers’ compensation cases (preferred).
• Recommended certifications (CCM, CRRN, CDMS, CRC); must obtain within two years of hire.
• Strong organizational, time‑management, PC, and communication skills (written, verbal, interpersonal).
• Ability to operate autonomously with minimal oversight.
• Proficiency in patient education.
• Valid driver’s license with good driving history.
