Provides professional nursing care for assigned patients utilizing the nursing process of assessment, diagnosis, planning, intervention, and evaluation, according to professional standards, and state regulations, and in keeping with the health center's mission in the clinic setting. Provide anticoagulation management under the direction of ambulatory care clinical pharmacist, physician, or advanced practice providers to improve compliance and effectiveness of anticoagulation therapy through in-person visits consisting of patient education, point of care INR blood testing, and a dosing protocol.
Supports the clinic care team by assisting with daily tasks, administering injections, conducting Medicare wellness visits, and providing triage support.
Graduate from accredited school of nursing or licensed practical nursing program
3 years clinic experience
Required Registration/License/CertificationCurrent Kansas Licensed Practical Nurse
Basic Life Support certification from either American Red Cross CPR/AED for the Health Care Provider or Basic Life Support (American Heart Association-Health Care Provider) successfully completed within the first 90 days of employment
Anticoagulation Therapy Management training (provided by SRHC). Training obtained within one year of hire date.
Minimal of 4 hours Continuing Education in the related field every 2 years.
