Social Services Director LCSW/LMSW – Midwest Health, Inc.

Join the Midwest Health team to lead our Social Services Department, ensuring residents’ physical, mental, emotional, and social needs are met in accordance with federal, state, and local standards.

Key Responsibilities
  • Comply with the facility’s personnel, safety, and corporate policies and procedures.
  • Report witnessed or reported allegations of resident abuse or neglect immediately to the Administrator/DON or Nurse Manager on Call.
  • Maintain confidentiality of all resident and family information.
  • Assist in planning, developing, organizing, implementing, evaluating, and directing all social service programs.
  • Meet with administration, medical, nursing, and related departments to plan social services; develop and maintain a quality assurance program and participate in facility surveys and inspections.
  • Coordinate admissions (readmissions, room moves, patient inquiries) and discharge planning, including the development of social care plans and resident assessments.
  • Arrange transportation to appointments, transfers to other facilities, and discharges as needed.
  • Interview residents and families privately and involve them in planning social service programs when appropriate.
  • Provide information on Medicare, Medicaid, and other financial assistance programs available to residents.
  • Prepare and maintain social services progress notes, assessments, and care plans.
  • Complete administrative requirements such as forms and reports and submit them to the Director.
  • Coordinate Medicare/Medicaid benefits, insurance benefits, and VA benefits for residents, including beneficiary consent, denials, and utilization.
  • Lead the social service department with authority, responsibility, and accountability; delegate duties to other personnel as necessary.
  • Maintain productive relationships with medical professionals and other health-related facilities and organizations.
  • Provide written and oral reports of social service programs and activities as required.
  • Attend department head meetings and review resident complaints and grievances, reporting actions taken to resolve them.
Physical Requirements
  • Regular worksite attendance.
  • Bachelor’s Degree required. Degree obtained from an approved school of social work preferred.
  • Licensure (LBSW) through approved state regulatory board preferred.
  • If required to operate a motor vehicle for business purposes, must successfully complete the requirements contained in Company’s Motor Vehicles policy.
  • Must have basic computer skills.
  • Must be able to read, write, speak, and understand the English language.
  • Must be able to work beyond normal working hours and on weekends and holidays when necessary.
  • Must be able to assist in the evacuation of residents during emergency situations.
  • Must be able to perform the essential position functions of the job with, or without reasonable accommodation.
  • Employee is required to use a transfer/gait belt or to obtain the assistance of another employee when attempting to assist residents with lifting or carrying objects in excess of 25 pounds.
