Role Description

The Family Centered Treatment Practitioner will focus on delivering compassionate care to at-risk children, youth, and their families by providing direct services, support, and intervention strategies. Responsibilities include conducting assessments, developing personalized treatment plans, working collaboratively with families, and assisting them in overcoming challenges. This is a full-time position that operates on-site in Salem, VA, providing an opportunity to make a meaningful impact locally.

Qualifications
  • Proficiency in Primary Care and Family Medicine, with the ability to address comprehensive physical and emotional needs of families.
  • Experience with Community Health and Urgent Care practices to effectively address immediate and ongoing family health needs.
  • Strong background in Medicine, particularly as it relates to supporting children, youth, and families in therapeutic environments.
  • Effective communication, problem-solving, and organizational skills to deliver family-centered solutions.
  • Previous experience working in a similar capacity or within the social services field is highly preferred.
  • Educational qualifications such as a degree in social work, counseling, psychology, or equivalent field are preferable.
  • Dedication to serving at-risk communities with empathy, integrity, and a team-oriented mindset.
