You are the heart of our clinical operations. You will bridge the gap between medical needs and holistic happiness.

Clinical Leadership: Lead, mentor, and inspire a diverse team of Caregivers, CNAs, HHAs, and LPNs. You set the standard for excellence.

Design Care: Conduct initial consults and ongoing assessments. You won t just check boxes; you will create personalized care plans that address medical needs and personal passions.

Quality Assurance: Oversee all clinical documentation, ensuring strict compliance with Oregon Health Authority (OHA) regulations and agency policies.

Collaboration: Serve as the primary liaison between families, physicians, and interdisciplinary partners to ensure seamless care coordination.

Growth & Development: Assist in recruiting top talent and provide hands-on training and mentorship to your staff.

• Experience: 2+ years of nursing experience. Background in Home Health, Hospice, or Geriatrics is highly valued.
• Leadership: 1+ year of supervisory or leadership experience. You know how to have tough conversations and how to celebrate wins.
• Knowledge: Familiarity with Oregon home health regulations and OARs.
• Requirements: Current CPR certification, negative TB test, valid driver’s license, and reliable transportation.
• Soft Skills: You are organized, a problem-solver, and an excellent communicator. You lead with empathy and execute with precision.
