Licensed Practical Nurse (LPN) – Home Health (Full-Time)

The Licensed Practical Nurse (LPN) provides skilled nursing care under the supervision of a Registered Nurse (RN) and in accordance with the patient’s individualized plan of care. The LPN collaborates with the interdisciplinary team to ensure patient-centered care that promotes health, healing, and safety in the home environment.

• Provide direct patient care in the home setting according to the plan of care established by the RN and physician.
• Administer medications and treatments as prescribed, documenting care accurately and timely in the EMR.
• Monitor and report changes in patient condition to the supervising RN and physician promptly.
• Perform wound care, catheter care, injections, and other nursing procedures as ordered.
• Educate patients and caregivers on disease management, medication administration, and home safety.
• Collaborate with other disciplines including physical therapy, occupational therapy, speech therapy, and social work to ensure coordinated care.
• Participate in case conferences, staff meetings, and in-service training.
• Maintain compliance with all federal, state, and company policies including infection control, HIPAA, and patient rights.
• Ensure timely and accurate documentation to support billing, compliance, and continuity of care.

• Current Oregon Licensed Practical Nurse (LPN) license in good standing.
• Minimum of one year of clinical experience, preferably in home health, medical-surgical, or geriatric care.
• Current CPR/BLS certification.
• Valid driver’s license, auto insurance, and reliable transportation.
• Strong organizational, clinical, and communication skills.
• Comfort with technology and electronic documentation.
