Aging and People with Disabilities (APD) is a program within Oregon Department of Human Services (ODHS). As an employee working within the Aging and People with Disabilities Program, you will assist older adults and people with disabilities of all ages to achieve well-being through opportunities for community living, employment, family support and services that promote independence, choice and dignity.
The Nursing Facility Survey Unit (NFSU) is part of the Safety, Oversight and Quality (SOQ) licensing program. NFSU is staffed by health care professionals who have been trained and certified by the Centers for Medicare & Medicaid Services (CMS) to conduct federally mandated survey and certification activities to determine compliance with requirements for long-term care nursing facilities (NFs) participating in the Medicare and Medicaid program and/or licensed in the State of Oregon.
As a Nursing Facility Client Care Surveyor, you will:
- Conduct survey work to evaluate the quality and adequacy of care and services received by residents of Nursing Facilities (NF) for substantial compliance within mandated federal regulations and state rules within required time frames.
- Work with a team of surveyors, either as a team member or a team coordinator, working both independently for some tasks and in a consensus process for others.
- Write reports of findings using specified CMS computer software program, and according to CMS “Principles of Documentation” guidelines. Reports must be clearly descriptive of findings, concise, complete, and legally defensible.
- Conduct intake/triage work to capture, review, clarify, and augment through interview and record review all relevant data received for NF public complaints and facility reported incidents using the federal database.
- Monitor all incoming NF complaints and incident reports.
- Create new intakes and enter all complaint/incident intake information into the CMS database.
- Contact complainant(s) to verify and confirm information received and solicit additional information to assist in determining potential noncompliance with federal and/or state requirements.
- Three (3) years of experience in areas related to nursing, social work, developmental disabilities, dietetics/nutrition, or sanitation;
AND one of the following: - a Bachelor's degree in a health or human services field;
OR - a license as a Registered Nurse.
- (LPN certification and/or experience will not meet this minimum qualification.)
- Current, active RN (Registered Nurse) license.
- Experience working in a long-term care setting as a licensed nurse, or a current Surveyor Minimum Qualifications Test (SMQT) certification.
- Experience conducting surveys or investigations in long term care settings.
- Demonstrated experience using critical thinking, independent judgment and decision-making.
- In depth knowledge of federal and state laws, rules, and regulations related to the provision of care and services in health or health-related settings.
- Experience adapting communication to diverse audiences regarding the application of laws, rules, regulations, policies, or procedures.
