• Perform evaluations and develop treatment plans.
• Regularly re‑assess effectiveness of treatment plans, attend staffing meetings and family conferences.
• Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources.
• Communicate patient needs and progress to the treatment team, physician, person receiving services and family members.
• Communicate with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes.

• Minimum of a bachelor’s degree in Physical Therapy from a college or university with an accredited PT program.
• Current, unrestricted license as PT in the state where practicing.
• Current Basic Life Support or Cardiopulmonary Resuscitation certification as required by state regulations.
• Minimum of one year’s work experience as a PT preferred.
• Demonstrates knowledge of rehabilitation techniques related to complex neurological injury.
• Communicates effectively and professionally in verbal and written interactions.
• Ability to lift 50 pounds, and handle transferring of patients up to 100 pounds following safety procedures.
• Other physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, positioning frequently at times; fine motor skills, visual acuity, walking/standing for extended periods.
• Health screen or examination may be required prior to assignment and periodically thereafter, depending on location policy.

Salary: USD $50.00 – $55.00 / Hour.
