Clinic HoursMonday – Friday | 7:00 a.m. – 6:00 p.m.
* Saturday | 7:30 a.m. – 5:30 p.m.
Sign-on Bonus: $21,000 (Internal candidates are not eligible)
Referral Bonus: $5,000 (expires at the end of this posting)
Salary Range: $212,888 to $266,163, up to $319,384, DOE
Provide culturally and linguistically appropriate medical services, including care coordination, working with a multidisciplinary team to support patients with complex needs. Contribute to an integrated care model addressing medical and behavioral health needs.

Responsibilities- Oversee patient care coordination, including referrals, case management, and enabling services.
- Provide direct medical care, diagnosis, treatment planning, and follow-up.
- Supervise nurse practitioners and physician assistants, and proctor medical students.
- Maintain accurate medical records, meeting organizational standards.
- Support organizational initiatives and collaborate across disciplines.
- MD/DO with completion of Primary Care residency, board certified.
- Licensed in Oregon with DEA and prescriptive authority.
- Ability to provide obstetrical care preferred.
- Strong interpersonal, communication skills, and team orientation.
- Valid Oregon Driver’s License, CPR certified, bilingual in English/Spanish preferred.
- Experience in community health is a plus.
- Ability to work independently, flexible hours, and take call.
