RN - Ambulatory Care

We are seeking a passionate and dedicated RN to join our client, a non-profit NCQA Level-3 Patient Centered Medical Home in Salem, Massachusetts. This is an exciting opportunity for someone who is looking to make a positive impact in the community and advance their career in family medicine. As a qualifying HRSA site for loan repayment, this position offers a unique opportunity to serve an underserved community and make a difference.

Responsibilities

• Provide high-quality nursing care to patients in an ambulatory setting
• Collaborate with the healthcare team to develop and implement patient care plans
• Perform patient assessments and document patient care in an EMR (preferably EPIC)
• Educate patients on preventive care and self-management strategies
• Assist providers with procedures and treatments as needed
• Participate in quality improvement initiatives to enhance patient care
• Communicate effectively with patients, families, and healthcare team members

Qualifications

• Current RN MA License or ability to obtain reciprocity in the state
• At least one year of experience working as a licensed RN in an ambulatory setting (CHC experience preferred but not required!)
• Current CPR certification or ability to obtain within thirty (30) days
• High level of commitment to continuous improvement
• Fluency in either Spanish or Portuguese is preferred
• Experience working with Microsoft Office
• Familiarity with EPIC (EMR) is preferred
• Enjoyment of a fast-paced environment
• Dedication to working with an underserved community
