Certification Details
- As a Home Health RN, responsibilities include assessing patient conditions, implementing care plans, and providing ongoing education and support to patients and their families.
- Patient care is delivered within organizational philosophy, policy and standards of community health practice.
- Perform admission processes and case management responsibilities in accordance with MGB Home Care’s policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications.
Job Details
- 1 year Certified Home Care experience required.
- 1 year Case Management Experience required.
- Ability to travel to cases as assigned.
Job Requirements
- Weekend rotations are once every 6 weeks.
Schedule Information
- MGB Badge Photo required for onboarding (color photo taken in past 6 months, no selfies, plain background).
- MGB MVR Check required within 45 days of start for all home care positions.
- License/Certificate Verification required for submission; primary source verification of professional license and/or certification within 30 days of start date; MA Conditional Licenses not accepted.
- Fit Test (N95) required within 365 days to start date and updated annually; no religious exemptions.
- MGB Nurse Aide Registry check required within 30 days of start as per MA DPH.
- Documentation of MMR vaccination or immunity required.
- Influenza Vaccine required during flu season (Sept 15 - April 30); no exemptions.
- TB Test documentation required within 3 months of start date and updated if >6 month break.
- Health Statement signed by MD/DO/NP/PA required within 365 days of start date and updated if >6 month break.
- MGB Criminal Background Check required within 45 days prior to start date and updated if >6 month break.
- MGB Massachusetts CORI required within 30 days prior to start date and updated if >6 month break.
- MGB Resume including past 7 years of work experience required for submission.
- MGB Skills Checklist required for submission.
- MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions.
- Respirator Medical Clearance Attestation required if fit test is required.
- MGB Education Verification required for onboarding.
- MGB SSN Trace required with criminal check.
- MGB National Sex Offender Check required within 30 days of start and updated annually.
- MGB Exclusion Checks required within 30 days of start and annually.
