Lifecare Home Health - - - Responsibilities:
Assist clients with personal healthcare and health maintenance activities in their place of residence.;
Communicate effectively with administrative staff regarding client/family needs assessments, including reporting any significant changes in client condition.;
Provide care as outlined in the Home Health Aide Care Plan and accurately document the cares provided.;
Contribute to continuity of care and effective communication between all care team members, and participate in Case conferences/Team Meetings.;
Apply infection control and universal precautions in client care activities.
