- Active Florida License - OR -
- Home Health Aide Certification minimum 40 hours (AHCA approved)
- Valid Photo
- Current Adult CPR Certification
- AIDS Bloodborne Pathogens Certification
- Assistance with Self‑Administration of Medication Certification
- Health Statement (free from communicable disease)
- Level 2 Background Screening
- Liability Insurance
