Clarendale of St. Peters is recruiting for a hospitality focused Health and Wellness Navigator to join our team!
Salary $75,000.00
Here are a few of the daily responsibilities of a Health and Wellness Navigator:
- Oversees cross‑functional departmental support of all post‑acute services within the community.
- Assists residents to transition within the continuum of care services to the appropriate levels of care offered within the community (Independent, Assisted, and Memory Care). Assistance provided by introducing community resources and the supportive services needed to obtain optimal levels of health.
- Coordinates/attends relative community meetings related to resident transitions, status updates, change in resident condition, support services required, and appropriate level of care including but not limited to:
- Daily Assisted Living人人爽人人 Memory Care Stand Up Meeting
- Daily Sales and Marketing Pre‑Sales Meeting
- Daily Director’s Meeting

- Weekly Collaborative Care Team Meeting
- Independent Living Resident Health Committee
- Independent Living Resident manana Counsel
- Support Groups
Here are a few of the qualifications we need you to have:
- Three to five years of experience working in the Senior Living Space, Independent Living, Assisted Living, and Memory Care or other health care setting with strong organizational skills, management experience with care coordination and care management with an interdisciplinary approach to care preferred.
- Graduate of an accredited college with.
Atoi LPN, RN, or bachelor’s degree in social services, geriatrics, nursing, and or related senior living services preferred.
