Overview

Responsibilities- Provide case management services to individuals with disabilities on CADI waivers, Traumatic Brain Injury Waivers, and Elderly Waivers.
- Coordinate services to meet clients’ needs and support independence in their communities.
- Engage in phone and email communication with clients, other service providers, family, and community members; conduct in-person visits with clients on caseload (with virtual visits possible when necessary).
- Ensure timely documentation of communications and meetings.
- Participate in training to understand county, state, and federal resources to meet clients’ needs and promote independence.
- Compassionate and empathetic individuals with strong listening, organization, and documentation skills.
