We are seeking compassionate and empathetic individuals to provide case management services to individuals with disabilities on CADI waivers, Traumatic Brain Injury Waivers, and Elderly Waivers. The successful candidate will have strong listening, organization, and documentation skills. Case Managers are responsible for coordinating services for individuals in order to meet their needs and work alongside people in maintaining as much independence as possible in their communities.
Responsibilities- Provide case management services to individuals with disabilities on CADI waivers, Traumatic Brain Injury Waivers, and Elderly Waivers.
- Coordinate services for individuals to meet their needs and help them maintain as much independence as possible in their communities.
- Communicate by phone and email with clients, other service providers, family members, and community members.
- Conduct in-person visits with individuals on their caseload twice yearly (currently temporarily done virtually due to COVID).
- Ensure timely documentation of communications and meetings.
- Receive training to familiarize with county, state, and federal resources to meet client needs and utilize available resources for independence.
