Job Title: Licensed Practical Nurse (LPN) – Skilled Nursing Facility

Location: Highland Chateau Health + Rehabilitation

Department: Nursing

Reports To: Director of Nursing

Job Type: Full Time, Part Time, PRN

Shift: Days, Evenings, Overnights

Job Summary:

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our skilled nursing team. The LPN plays a critical role in delivering high-quality care to residents by supporting daily nursing operations, coordinating with interdisciplinary teams, and ensuring each resident's individualized care plan is followed with excellence and empathy.

Key Responsibilities:
  • Provide direct nursing care to residents in accordance with their care plans

  • Administer medications and treatments as prescribed by physicians

  • Monitor residents’ conditions and report any changes to the RN or physician

  • Document all nursing interventions and resident responses accurately and promptly

  • Assist with assessments, admissions, discharges, and transfers of residents

  • Collaborate with CNAs and other staff to ensure the highest quality of care

  • Maintain a clean, safe, and supportive environment for residents

  • Provide education to residents and families regarding health conditions and care needs

  • Uphold all state, federal, and facility regulations and standards of care

  • Participate in ongoing training and quality improvement initiatives

Qualifications:
  • Current and valid LPN license in the state of Minnesota.

  • Graduation from an accredited practical nursing program

  • CPR/BLS certification

  • Long-term care or skilled nursing experience preferred

  • Strong clinical, communication, and critical thinking skills

  • Compassionate, reliable, and team-oriented attitude
