Responsibilities
  • Provide professional, quality program services to the clients served by the Youth ACT team (individuals aged 14-20 years) and all other RADIAS Health programs.
  • Promote the physical and mental health of clients on the Youth ACT team by monitoring, educating, and coaching clients as needed.
  • Work with other members of the treatment team to assist clients to obtain and maintain an optimum quality of life.
  • Provide nursing leadership for all aspects of client care.
  • Take the lead role and participate in providing medication administration, education and medical services.
  • Have knowledge of operational expectations, policies and procedures for each assignment area.
  • Develop, maintain and supervise team psychopharmacologic and medical treatment and medication policies and procedures, including administering, evaluating and recording psychotropic medications prescribed by the team prescriber (Psychiatrist or APRN).
  • Evaluate and chart psychotropic medication effectiveness, complications and side effects, and arrange for required lab work according to protocol.
  • Participate in daily team meetings and treatment planning meetings.
  • Manage the system of distribution of medication to clients and integrate medication administration into clients’ individual treatment plans.
  • Administer injectable medications to clients using best practice.
  • Routine supervision with Team Lead and the Director of Nursing.
  • Actively and assertively engage with clients and their natural supports.
  • Manage pharmaceuticals and medical supplies.
  • Maintain a record of controlled medications and samples and manage the destruction of outstanding medications.
  • Coordinate, schedule, and administer medical assessments of client physical health, make appropriate referrals to community physicians, and coordinate psychiatric treatment with medical treatment providers to ensure continuity of care.
  • Work with the prescriber to coordinate care with external providers to encourage consistent and appropriate ongoing care of the client.
  • Assess and establish appropriate medication management plans for clients, including obtaining necessary prescriptions, medication administration, and monitoring side effects. Promote skill building for medication self‑management.
  • Provide cross‑training to teams on topics related to the integration of mental health/physical health and medication.
  • Support the medication administration certification process for RSS Mental Health Counselors, IRTS staff, and any other appropriate staff, as needed.
  • Work with clients on all aspects of psychiatric rehabilitation, support, and psychotherapeutic interventions identified in the treatment plan.
  • Take and document vitals on clients as appropriate.
  • Process all prior authorizations in collaboration with prescriber.
  • Keep appropriate records and do necessary charting as required.
  • Supervise all aspects of medication administration.
  • Treat medical problems requiring nursing judgments and coordinate appropriate follow‑up.
  • Transcribe all physicians’ orders on the Physician Order Sheets.
  • Coordinate OSHA regulation requirements.
  • Provide courteous and respectful care to all clients.
  • Work effectively and professionally with other staff members.
  • Review all medication errors and narcotic off counts as relevant to program requirements.
  • Arrange for biohazard disposal, medication audits and lab tests if needed.
  • Provide education or refer clients to educational resources for optimal health.
  • File reports with Vulnerable Adult, the Ombudsman's Office, Child Protection, and other mandated/required reports when appropriate.
  • Participate in evening, weekend, and holiday coverage and be on‑call as needed by the team.
