Overview
We are a mobile, facility-based primary care practice serving residents in SNFs, LTCs, and ALFs, focusing on high-quality, patient-centered care.
Responsibilities- Provide full-scope primary care, manage acute and chronic conditions, and build trusted relationships with residents, families, and care teams.
- Conduct evaluations, interpret tests, diagnose, prescribe medications, educate stakeholders, and ensure coordinated care.
- Active Minnesota NP license, board certification, and preferably experience in geriatrics or skilled nursing settings.
- This full-time role is based in Grand Rapids, MN, with in-person work supporting the delivery of innovative post-acute and long-term primary care.
- Compensation: $100,000–$140,000 salary
- Sign-on: $15,000
