SUMMARY

The MDS Coordinator, RN supports the applicable clinical software used in assessments and care

planning, with a goal of facilitating quality assurance.

PRIMARY RESPONSIBILITIES

• Oversee and administer the Resident Assessment process according to applicable policies and
• regulations.
• Facilitate the completion and updating of resident Care Plans within MDS and the IDT to reflect the resident’s most current needs.
• Coordinate skilled coverage for residents with managed care.
• Review Medicare Part A coverage and necessary documentation for eligible residents.
• Obtain signed physician certification forms as indicated.
• Participate in quality improvement initiatives as directed.
• Provide necessary documentation of all activities to meet regulations.
• Support various other Nursing responsibilities as needed.

QUALIFICATIONS

• Licensed as a Registered Nurse in the State of Minnesota and must maintain licensure.
• One or more years’ nursing experience in a long-term care setting required.
• One year of MDS and care plan development experience preferred.
• CPR Certification preferred.

Compensation
:
An individual's pay rate within the posted range may be determined by various factors, including skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization prioritizes pay equity and considers internal team equity when making any offer. Hiring at the maximum of the range is not typical. Shift and/or other differentials may be available to increase your pay rate for certain shifts or work.
