We are seeking an experienced Nurse Practitioner to provide comprehensive primary care in skilled nursing, long-term care, and assisted living facilities.
Responsibilities- Perform patient assessments, manage acute and chronic conditions, and develop treatment plans.
- Order and interpret diagnostics, prescribe medications, and educate patients, families, and staff.
- Collaborate with care teams to ensure seamless, patient-centered care.
- Maintain accurate documentation and support quality improvement.
- Current MN NP license, board certification.
- 1+ year experience preferred.
- Strong geriatric and primary care skills.
