Registered Nurse (RN) Triage - Per Diem

The Triage Registered Nurse is an experienced nurse responding to overnight and weekend calls regarding the changing needs of hospice patients. The nurse answers questions, gathers information, assesses patient needs and, when needed, coordinates and directs hospice services and care via phone conference. This role requires a focus on palliative care.
Shift schedule: 8 am to 8 pm every other Saturday and Sunday.

Qualifications- Must have a current Registered Nurse (RN) license in the state(s) of practice.
- For nurses holding a multistate license, a compact license may cover practice in other states without obtaining additional licenses while maintaining the RN license in the primary state of residence (note - Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact).
- Possess or obtain an RN license for the 10 states served (MN, WI, IA, SD, IL, MI, MO, IN, KS, NE) or a compact license covering these states before hiring.
- Graduate of a professional school of nursing and currently licensed, certified, or registered in the state(s) of practice.
- Ability to pass DHS background, consumer report, and drug test.
- If driving:
Valid State Driver’s License, automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record.
- At least 1 year of recent hospice/home care experience.
- At least 2 years of general nursing experience.
- Performs all duties and responsibilities in accordance with the Nurse Practice Act and professional nursing guidelines.
- Receives patient‑care calls and instructs patient/caregiver on appropriate interventions based on nursing knowledge.
- Evaluates available options and uses critical thinking to select the most appropriate intervention.
- Determines the need for a nursing visit based on EMR and telephone call; dispatches a visit RN or LPN as appropriate.
- Completes, submits, and maintains accurate and relevant clinical notes regarding patient condition and care provided.
- Communicates with the physician regarding patient needs and reports changes, obtains physician orders as required.
- Assumes responsibility for personal growth, maintaining and upgrading professional knowledge through continuing education and in‑service classes.
- Actively participates in quality assessment and performance improvement teams.
- Other duties as assigned.
- Facilitates patients’ efforts toward self‑sufficiency and optional comfort care.
- Counsels and collaborates with family/caregiver and interdisciplinary team to meet emotional needs of the patient and family.
- Ensures arrangements for equipment and other necessary items and services are available.
- Maintains compliance with HIPAA regulations, treating all patient information as confidential and using protected health information only as necessary.
The wages for this role are $360 per diem shift. Hourly rates apply only for orientation, meetings, and education, at $30 per hour. Final annual salary will be based on geographic location, education, relevant certifications, depth of experience, and special skillsets. The total compensation package may also include participation in employee benefits such as medical, dental, vision, pet insurance, disability, life insurance, paid time off, 401(k) retirement plans, subject to plan terms, and eligibility for other compensation.
