Quality Assurance Coordinator

Position Purpose

To provide support to all sites by facilitating quality assurance systems, including the coordination of investigations, monitoring compliance, and ensuring training and development programs are implemented. The Director of Quality Assurance and Training (DQAT) is responsible for strategic oversight, compliance, and staff orientation, ensuring high-quality residential and supportive services.

The DQAT reports to the Vice President of Community Services and ultimately to the President/CEO. Responsibilities include facilitating investigation processes, developing and executing corrective plans, supporting sites during state surveys, and continuously improving training programs. The role requires extensive writing, communication, organization, and management skills, as well as the ability to develop effective action plans and best practice recommendations.

• Manage overall program functioning, including programmatic and residential services.
• Facilitate program monitoring, develop programming and documentation systems, and ensure compliance with regulatory requirements.
• Respond to issues during non‑work hours and coordinate effective meetings.
• Develop and maintain quality of service standards in collaboration with the Vice President of Community Services.
• Evaluate services through chart audits, treatment observations, mock surveys, and other monitoring systems.
• Coordinate and facilitate effective meetings and provide clear memos/reports.
• Develop best practices and training for Person Centered Principles across all Phoenix programs.
• Lead investigations of Vulnerable Adult Reports, develop internal reviews, and respond to incidents.
• Oversee staff orientation and training, ensuring compliance with state and federal statutes.
• Participate in fundraising, fiscal integrity activities, and committee leadership as directed.
• Maintain a safe work environment, adhering to safety policies and conducting emergency responses.

• Bachelor's Degree in Human Services or related field (required).
• Three (3) or more years of supervisory or management experience in long‑term or residential settings.
• Qualified Developmental Disability Professional (QDDP) licensure.
• Experience with ICF/DD regulations preferred.
• Experience in training development and facilitation.
• CPR certification within first six months.
• Medications administration training within first six months.
• Drive Phoenix‑owned vehicles within first six months.
• 24‑hour coverage availability and responsive to issues.
• Excellent problem‑solving, decision‑making, organization, and communication skills.
• Ability to lift up to 75 pounds and operate wheelchair transport.
• Valid driver’s license with safe driving record.
• Full‑time employment commitment.

The role requires travel between sites, frequent standing and walking, desk work, extensive telephone communication, and the ability to provide direct care services as needed. Working hours extend beyond the standard 24‑hour coverage, and employees must be flexible for coverage requests.
