St. Anthony Park Home is seeking an experienced Admissions Director to manage admissions and census for our skilled nursing facility. This role owns the referral-to-admit process and works closely with nursing, social services, and the business office to ensure smooth and appropriate admissions.
Responsibilities- Manage all admissions from referral through move-in
- Coordinate with hospitals, families, and referral sources
- Maintain and grow facility census
- Review clinical and financial appropriateness of admissions
- Collaborate with interdisciplinary team
- Nursing home admissions experience required
- Skilled nursing / post-acute background
- Strong communication and organization skills
- $28–$35/hour, based on experience
- Full-time
