Job Summary

The Quality Improvement Manager supports, coordinates, and advances clinical quality improvement (QI) initiatives across Minnesota’s FQHCs. This role is responsible for implementing MNACHC’s QI programming, delivering technical assistance (TA), and supporting health centers in meeting federal requirements, statewide quality efforts, and emerging priorities.

This position plays a key role in implementing maternal health improvement initiatives to improve maternal care and outcomes across health centers. The QI Manager will also lead and support additional grant-funded projects as new opportunities arise. The role requires close collaboration with MNACHC staff, FQHC leadership and staff, and state and national partners.

Key Responsibilities

Quality Improvement Support & Technical Assistance

  • Implement QI initiatives aligned with HRSA federal requirements, MNACHC’s strategic plan, and specific health center needs.
  • Provide coaching and technical assistance on QI methods, tools, and best practices.
  • Support health centers in meeting HRSA requirements related to:
  • QA/QI plans
  • UDS clinical quality measures
  • Assist health centers in designing, tracking, and evaluating QI projects.
  • Partners with Health Center Controlled Networks to provide coordinated support to health centers.

Maternal Health Initiatives

  • Lead or support the implementation of MNACHC’s maternal health quality programs.
  • Provide maternal health–specific resources, data support, and TA.
  • Collaborate with external partners working to improve maternal and perinatal health statewide.
  • Assist health centers in assessing and addressing intimate partner violence.

Training, Education & Convening

  • Develop and deliver QI-focused webinars, workshops, and resource materials.
  • Facilitate peer learning communities or round tables for FQHC quality and clinical staff.
  • Contribute to the planning and execution of MNACHC’s annual Many Faces of Community Health Conference.

Data, Reporting & Analytics

  • Provide benchmark data, performance summaries, and data interpretation guidance to health centers.
  • Support health centers in understanding and meeting state and federal reporting requirements.
  • Assist teams in using data to inform QI strategies and decision-making.

Grant & Project Management

  • Partner with MNACHC leadership to lead the implementation of grant-funded initiatives as they are assigned.
  • Ensure timely reporting, data collection, documentation, and communications with funders.
  • Support proposal development for new grants and opportunities.

Partnership & Representation

  • Represent MNACHC on work groups, committees, and collaborative teams related to QI.
  • Collaborate with state, regional, and national partners to advance quality efforts.
  • Support policy staff by monitoring regulatory, legislative, and policy changes that affect clinical quality.
Qualifications

Required

  • Bachelor’s degree in a health-related field (nursing, public health, health administration, health informatics, etc.) OR equivalent experience in healthcare quality improvement.
  • Experience implementing QI initiatives in clinical, community health, or primary care settings.
  • Knowledge of QI methodologies (e.g., PDSA cycles, root cause analysis, workflow redesign, Lean Six Sigma, etc.).
  • Strong communication skills, including facilitation, presentations, and written materials.
  • Ability to build relationships and work collaboratively with key partners.
  • Ability to travel within Minnesota and occasionally out of state.

Preferred

  • Experience working in FQHCs or other safety-net health care settings.
  • Familiarity with HRSA/BPHC requirements.
  • Experience with maternal health or perinatal health improvement initiatives.
  • Experience with health data analytics and EHR systems.
  • Experience managing or implementing grant-funded projects.
