Position: Programming and Community Coordinator - St. Paul, MN - 1800 Sign-on Bonus

Overview

$1,800 sign-on bonus payable over the first six months of employment.

Service provision for persons served
  • Facilitate day to day programming options for persons served. Ensure that people receive programming options that are person-centered, age appropriate and community focused.
  • Act as the designated programming coordinator for the center. Meet with individuals and groups to find out what they want to learn about. Use that information to develop programming within the center.
  • As appropriate work cooperatively with other MSS staff and community employers to ensure that the person served has a successful outcome.
  • Provide day-to-day program coordination and services for persons served. Ensure that people receive services that are person-centered, age appropriate and community focused.
  • In cooperation with other Support Team members, develop, record and implement CSSP-A's, SMA's, IAPPs, ISP's and other plans in a timely manner. Attend and participate in the Support Team Meetings for assigned persons. Maintain contact with other Support Team members regarding changes in plans, concerts, incident reports, medical changes, etc.
  • Maintain up-to-date files for assigned individuals including the MSS Client Database. Ensure that other program records, i.e. attendance records, production related records, electronic data records, etc. are completed in an accurate and timely manner. Ensure that all individual program records are distributed to other Support Team members as requested/required.
  • Review the medical needs of assigned individuals with the Support Team Members and Nurse Consultant. Ensure that any medical needs of persons served are being met in a timely manner.
  • Assist persons served with their personal needs as necessary, including using the bathroom, eating, dressing, etc.
  • Abide by and implement all agency policies and procedures that pertain to the operation of the program, including but not limited to Rule 245D standards, Data Privacy, Emergency Use of Manual Restraints, VA Policy and Procedure, etc.
Teamwork and Communication
  • Communicate program plans, medical needs, etc. to Direct Support Professionals, and other direct service staff so that each person in the program receives the services that have been agreed upon with the Support Team. Provide guidance and instruction whenever needed to the direct service staff on the implementation of these plans and procedures. Instruct direct service staff in day to day expectations for maintaining daily documentation.
  • In cooperation with the Program Director, Program Supervisor and other Service Coordinators, plan and implement person-centered individualized and group activities within the center and in the community.
  • Work cooperatively with other Service Coordinators and assume duties of other Service Coordinators in their absence as directed by the Program Director.
Safety and Transportation
  • Secure and maintain medication certification and be responsible for all aspects of medications as assigned. Secure and maintain CPR and First Aid certifications and provide emergency care to persons served when needed.
  • Act as a liaison between MSS and residential providers in transportation concerns for persons served.
  • Ensure that MSS drivers and contracted drivers have the emergency information needed for persons served that they transport.
  • Ensure the safety of persons served and coworkers by following all safety related policies and procedures, including Emergency Procedure Plans, Health Care Manual, etc. Promptly report any unsafe conditions to the Program Director.
Requirements

Must possess the educational and experience necessary to provide quality services:

  • A BA/BS degree in a related field plus 1 year of work experience providing direct care for persons with disabilities; or
  • An AA degree in field related to human services plus 2 years of work experience providing direct care for persons with disabilities; or
  • A diploma in a field related to human services from an accredited postsecondary institution and 3 years of full-time work experience providing direct care services to persons with disabilities; or
