Position purpose

The Behavioral Health Direct Care Support Professional provides direct support to individuals experiencing significant mental health challenges. This person works direct care hours with individuals assigned, giving one-on-one care. This role involves developing an understanding of each person’s care plan through review of assessments and implementation of those care plans. At The Phoenix Residence, Inc., you’ll build relationships, provide person-centered, quality living experiences, and support individuals with disabilities in need of love and care.

Your goal is to help individuals enjoy life by helping them perform their daily activities, including meal preparation, light exercise, local transportation, social interactions and housekeeping. Your compassion and diligence as a support professional will help clients feel encouraged, involved and valued.

Responsibilities
  • Work with individuals experiencing mental health challenges as identified by the administrative team.
  • Understand each individual’s needs and develop updated strategies to be implemented by the support team.
  • Plan, perform, or assist with cares or activities according to each person’s preferences or needs (household tasks, community involvement, social, recreation and leisure activities, communication, personal safety, and behavior support).
  • Implement Positive Support Strategies contained within a person’s care plan; update and develop strategies as appropriate; provide training where needed.
  • Adhere to the Positive Support Rule and use a Person-Centered approach.
  • Document all activities and plan adjustments per Phoenix procedures.
  • Provide direction and support for individuals with mental health diagnoses, including training for staff and acting as a liaison with behavior and mental health professionals.
  • Advocate for the needs of residents within the home.
  • Identify and respond to emotional and social needs; interventions do not constitute psychotherapy.
  • Perform tasks that fall within Category II of infection control, including exposure to blood, body fluids or tissue as applicable.
Safety
  • Adhere to all safety policies and procedures; promote a safe living and working environment.
  • Follow lifting and transfer guidelines; train others in proper lifting techniques and body mechanics.
Regulation and Compliance
  • Understand and comply with PRI policies, procedures, and philosophy, and with licensing agency rules and regulations.
  • Possess working knowledge of applicable rules and regulations (e.g., ICF/ Regulations, Rule 245D, Rule 10, 185, Positive Support Rule, Supervised Living Facility Regulations, Life Safety Code, Food and Beverage, funding rules and employment laws).
Development of Position
  • Attend all mandatory in-services and meet annual training requirements.
  • Participate in continuing education as assigned.
Customer/Human Relations
  • Communicate effectively with individuals, staff, supervisors, families, interdisciplinary team members, and other customers in writing and verbally.
  • Demonstrate cooperative, supportive, and professional conduct; model The Phoenix Residence values.
  • Maintain confidentiality and adhere to PRI Guiding Principles of Person Centeredness, Teamwork, and Professionalism; show flexibility, responsibility, and open communication.
Qualifications
  • High School Diploma required.
  • Certification:
    Mental Health Technician/Behavioral Health Technician required.
  • Ability to assist with Qualified Intellectual Disabilities Professional (QIDP) duties as needed.
  • Mental health experience, including development and implementation of behavior and/or mental health plans.
  • CPR, First Aid, and Trained Medication Passer certification within the first 60 days of employment.
  • Valid driver’s license with a safe driving record; able to drive Phoenix-owned vehicles within the first 60 days of employment.
