ACT Case Manager – Bilingual Hmong at Wilder
Compensation: $52,000 – $56,000 per year. This is a full‑time, location‑specific position based in St. Paul, MN.
DutiesJoin our dedicated Assertive Community Treatment (ACT) Team as a mental health case manager, where you will provide comprehensive, community‑based mental health services. You will work directly in the community to empower clients, coordinate essential services, and foster long‑term stability and recovery.
- Provide rehabilitative treatment, case management, and life‑skills coaching.
- Serve as a mandated reporter for abuse and neglect.
- Conduct functional and risk assessments and develop individualized treatment plans.
- Maintain accurate and timely clinical documentation and reporting.
- Coordinate care with psychiatrists and other healthcare/community providers.
- Connect clients with community resources and build partnerships with other agencies.
- Provide community outreach and facilitate support groups.
- Ensure clients maintain contact with their psychiatrist and adhere to prescribed medications.
- Actively participate in team meetings and collaborate with the multidisciplinary team.
- Bachelor’s degree in Behavioral Science or related field (experience does not substitute for education).
- Minimum 1 year of experience in family outreach, support, care coordination, counseling, or community mental health.
- Must meet local mental health practitioner requirements.
- Bilingual in English and Hmong.
- Valid driver’s license, auto insurance, and reliable vehicle.
