Ramsey County Case Manager

Overview

Ramsey County Case Manager at Fraser – a non‑profit dedicated to autism, mental health, and disability services. We are building an inclusive, connected world where everyone belongs.

• Advocate for Fraser clients and connect them to community resources and services.
• Provide client‑centered case management for individuals receiving CADI/BI and DD waiver services.
• Maintain accurate documentation and case notes.
• Meet with clients two to three times per week; caseload averages 35–43 clients.
• Travel to meet with clients as required.

• Bachelor’s Degree (minimum) in social work, special education, psychology, nursing, or related field. If degree is in social work, social work licensure is required.
• One year of experience with developmental disability (DD) populations, or willingness to complete the Relias I/DD course (paid by Fraser).
• Experience or strong interest in working with adults with serious and persistent mental illness.
• Commitment to promoting diversity, multiculturalism, and inclusion with culturally responsive practice.
• Valid Minnesota Driver’s License with acceptable record.
• Ability to pass DHS background study.
• Computer proficiency using Word and Excel; excellent organizational skills and attention to detail.

• Monday through Friday during standard business hours. Fully remote after 3–6 months onboarding is complete.
• Supports Ramsey County; travel required to meet with clients 2–3 times per week.
• Salary starting at $50,000.
