Role PurposeThe role of the Case Manager is to provide services that stabilize the client s condition, provide assistance and support to prevent hospitalization, and focus on the recovery process. Responsibilities include carrying out rehabilitation and support functions and assisting in treatment, education, support, and consultation to families, and crisis intervention in consultation with the multi-disciplinary team. This position engages clients and establishes trusting collaborative relationships toward the goal of competitive employment in community job settings with other workers without psychiatric disabilities.

- Coordinate integrated services and supports necessary to assist each client toward recovery.
- Provide collaboration and education to significant others and family members involved in the client s recovery.
- Recognize significant changes in client status, symptoms, safety risks, and life circumstances promptly.
- Collaborate with the Clinical Supervisor or Team Leader; prioritize client needs and intervene in crisis situations effectively.
- Develop an Individual Community Support Plan (or Individual Family Community Support Plan) to promote independence and well-being, including services such as benefits, housing, crisis prevention, and health monitoring.
- Ensure services are implemented as planned and continuously monitored and evaluated.
- Treat all clients with dignity and respect, considering diagnosis, age, gender, and culture.
- Document all changes in client status or needs.
- Follow mandated civil commitment procedures and timelines; inform about complaint and grievance procedures.
- Maintain client confidentiality and privacy in line with MHR policies and HIPAA/MN Medical Records Act.
- Meet productivity standards (e.g., outreach, caseload, face-to-face contact, AIMs, charting, data entry) as outlined by leadership.
- Provide coverage to ensure continuity of care for each client served.
- Participate in all scheduled staff meetings and trainings unless excused by the supervisor.
- Bachelor s degree in social work, psychology, or human services from an accredited college or university; meets requirements for supervision and continuing education.
- At least 2,000 hours of supervised experience in delivering mental health services to children.
- Skilled in identifying and assessing a wide range of recipient needs.
- Knowledgeable about local community resources and how to utilize them for the recipient s benefit.
- Without a bachelor s degree, one of the following: 3-4 years of experience as a case manager associate OR a registered nurse with 3+ years of specialized psychiatry training and relevant community involvement or discharge planning in a mental health setting.
