The Healthy Aging Systems Planner leads strategic planning and implements activities to educate providers, improve clinical practice, and engage stakeholders in brain‑health initiatives. The role also supports epilepsy programming and provides strategic expertise for age‑friendly health‑system strategies, clinical‑community linkages, and aging‑related social determinants of health.
This position is temporary and could end at any time; the anticipated end date is 09/29/2028.
- Three (3) years of experience within public health, chronic disease prevention, or health promotion.
- Experience counseling and/or providing referrals for adults at risk of or receiving care for a chronic disease.
- Project‑management experience, including designing work plans, offering solutions, evaluating results, and conveying outcomes to stakeholders.
- Experience working with multidisciplinary teams from diverse backgrounds.
To be assessed at interview:
- Knowledge of how policy and systems changes affect quality, delivery, and accessibility of clinical services.
- Ability to edit, organize, and synthesize information from diverse sources into public‑health communications, professional‑education resources, plans, and reports.
- Knowledge of Alzheimer’s disease and related dementias.
- Knowledge of Age‑Friendly care principles and models.
- Knowledge of quality‑improvement processes in chronic disease prevention and management.
- Knowledge of the Department of Health’s organizational structure, methods, and procedures for complex projects.
Successful completion of a background check and employment reference check is required. A valid driver’s license is not required, but travel is expected and the candidate must be able to meet travel requirements.
