Description

Using a person-centered, strengths-based perspective, the Wellness Case Manager provides care coordination with an emphasis on mental health care and substance use recovery to victim-survivors who have been directly affected by domestic violence. The Wellness Case Manager supports residents in the Shelter Program, as well as participants in the community-based Holistic Wellness Program.

The Wellness Case Manager assesses individual needs, develops a unique care plan, and coordinates services like mental health, substance use recovery, housing, medical care, vocational support, and social services. Emphasis is placed on engaging participants in the delivery of services through multidisciplinary teamwork and coordination.

This position is full-time with some days required on-site. The annual salary for this position is budgeted at a range of $49,000 - $52,000.

Primary Responsibilities
  • Evaluates individual’s strengths, needs, goals and works collaboratively with the individual to develop a personalized support plan.
  • Connects residents with needed services, including mental health care, physical and dental health, vocation assistance, daily living assistance, financial assistance, and substance abuse recovery.
  • Acts as a liaison between the individual and other providers (doctors, therapists, social workers) to ensure all needs are being addressed.
  • Refers residents to other community resources when needed and follows up to ensure they have been connected to the resource.
  • Facilitates a weekly support group for shelter residents, structuring sessions around themes designated by the wellness team.
  • Engages and maintains relationships with community service providers, state and local agencies, and healthcare providers to coordinate care.
  • Engages a multi-disciplinary team including Women’s Advocates care teams and wellness team to provide a collaborative response to service delivery.
  • Completes and maintains clinical and case documentation in electronic records, including statistical data in accordance with HIPAA, organization, and program standards.
  • Other duties as assigned.
  • Requirements Required Skills and Experience
  • Three to five years of experience in a behavioral health field, preferably with individuals and /or families who have experienced domestic violence.
  • Experience as a case manager, or other related role.
  • Experience working with individuals and families who have experienced trauma.
  • Knowledgeable of local community mental health and substance use resources.
  • Ability to prioritize, handle a variety of tasks and be self-directed.
  • Ability to collect and maintain confidential data.
  • Ability to work effectively across differences.
  • Demonstrated experience with and/or commitment to intercultural development and anti-racism work in the workplace.
  • Commitment to actively contributing to the organization being a safe space for members of the LGBTQ+ community.
  • Excellent organizational skills, ability to manage multiple tasks and meticulous attention to detail.
  • Physically ambulatory and able to negotiate multiple stairways.
  • Computer literacy (Microsoft 365, SharePoint, Teams, etc.).
  • Valid driver’s license with current insurance and a reliable motor vehicle.
  • Criminal background checks are required for this position.
  • Preferred Skills
    • Experience in mental health case management.
    • Experience working with individuals in recovery.
    • Knowledgeable about systems of violence and trauma, including inter-generational effects of historical trauma.
    • Ability to perform CPR, first aid, and respond to emergency situations to include administering Narcan/Naloxone.
    • Associates or Bachelors degree in humanities or a related field.
    • Second language preferred.
