Join our Minneapolis Metro team as a PRN Speech Language Pathologist. We have 8 SNF/Transitional Care Unit locations in Minneapolis, Bloomington, Burnsville, Edina, Lake City, and Wyoming, MN. As a Speech Language Pathologist, you will provide direct care to adult and geriatric patients, focusing on swallowing disorders, dysphagia, and communication disorders, in accordance with state practice guidelines.

• Provide screenings and evaluations to determine requirements for skilled speech therapy intervention related to patients’ functional level.
• Develop and implement treatment plans focused on functional and measurable outcomes, based on physicians’ orders and professional standards.
• Monitor patients’ progress and adjust treatment plans as needed.
• Maintain accurate and up‑to‑date patient records.
• Collaborate with, coordinate, and participate in training programs for community staff, patients, families, and rehabilitation staff.
• Participate in continuous quality improvement activities.

• Successful completion of an accredited Speech Language Pathology program.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong problem‑solving and critical thinking skills.
• Compassionate and patient‑centered approach to care.
• Current valid Speech Language Pathologist license to practice in the appropriate state.
