Qualifications- BS/BA in social work, psychology, or related field.
- 2 years of experience working in social services is required.
- Driver's license and current insurance required.
- Perform timely intake interview and develop Discovery and Exploration (D&E) Plan.
- Maintain case load size that is both programmatic and fiscally appropriate and ensures that D&E services are completed in a timely manner meeting agency expectation (average of 6‑10 weeks depending on program).
- Develop and maintain good working relationships with referral sources and internal team and provide ongoing communication regarding participant activities. Utilize referral information, intake information, job history, local employment market, career exploration tools to plan activities to identify strengths and support needs related to job interests.
- Complete all agency documentation in a timely manner and maintain confidentiality according to agency guidelines.
- Completes community exploration activities with participants to analyze areas such as learning style, work pace, prompts and reminders needed, communication style, interest in work being performed, comfort in work environment, accommodations needed, etc.
- Actively work with participants and relevant support persons to obtain and incorporate feedback related to D&E recommendations, including life skill needs and social supports.
- Write thorough final reports with recommendations for employment next steps.
- Maintain average of 3+ new starts each month.
- Maintain minimum of 90% of participants completing assessment.
- Other duties as assigned.
