St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St.
Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.
Catholic Charities of the Archdiocese of St. Louis, service line St. Patrick Center, is looking for a mission and values driven individual to join our at our off‑site location, Caritas St. Matthews. The Case Manager of the Contracted Emergency Residential Services (CERS) Program is part of the Grant Per Diem (GPD) program and will utilize a wrap‑around philosophy and techniques to assist clients in transitional housing, meet with clients to maintain family and natural supports in the community, collaborate with clients and track success of their treatment plan, coordinate individualized services and the involvement of providers working in the home and community setting, as well as document all support related to activities and services.
This position requires a valid driver's license, personal vehicle, and auto insurance. This program is a 24/7 program and requires the ability to consistently work Monday through Friday, 11am to 7:30pm with the ability to remain flexible with scheduling.
- Visit clients weekly to track ISS goals and screen for suicidal and homicidal risks.
- Transport clients to community partner appointments.
- Contribute to the ideas and processes of the case management team.
- Use a peer-to-peer approach to build rapport with individuals in recovery.
- Provide emotional and informational support for permanent housing by assessing strengths, desires and immediate and long-term needs of each client.
- Develop or contact family and natural supports which exist in the community.
- Receive input from case management team members regarding employment, housing, mentoring, substance use, other immediate/long term needs, and long-term financial stability needs.
- Complete daily and monthly departmental paperwork.
- Daily update of electronic files including contact with clients, services or any internal or external referrals made.
- Adhere to the strategies and goals outlined in the ISS and Treatment Plan
- Coach clients to make solid well-informed decisions which maintain housing and advocate for the client.
- Assist client with monthly budgeting.
- Complete treatment plans and track goals monthly.
- Provide financial and money management counseling sessions.
- Provide weekly case management sessions to ensure service delivery.
- Teach Life Skills classes.
- Refer client to additional resources to address other service needs (i.e., legal assistance, VA benefits, community substance use counseling etc.)
- Research additional benefits available to enhance service being provided.
- Engage the client, the family and natural supports within the community setting when feasible and desired by client.
- Network with federal, state, and local agencies.
- Administer Waived Testing - drug urinalysis testing.
- Any other duties as assigned.
- Minimum of an Associate’s level education in social work or other human services related field or at least two years’ experience providing homeless case management services.
- First Aid Training and CPR Certification within 30 days after hire.
- Knowledge of housing and other supportive service resources.
- Knowledge of Housing First and Harm Reduction.
- Requires driver license (MO residents require Class
E). - Requires personal vehicle and auto insurance.
