Schedule: Full time, 7a–7p varying nights with every 3rd weekend, holiday requirement, no on‑call requirement

Job Summary

Pre‑licensure graduate of an accredited nursing program who actively participates in the SSM Health Saint Louis University Hospital New Graduate Nurse Residency Program. Under the direction of an RN preceptor, the nurse provides care to patients within scope of role and professional standards, delivering age‑appropriate care for assigned patients with a wide variety of medical conditions.

Responsibilities
  • Perform comprehensive nursing assessment/reassessment.
    • Age‑appropriate admission or transfer assessment, obtaining family/guardian input when appropriate.
    • Accurate and complete documentation of findings.
    • Assessment of post‑op/post‑invasive procedure patients.
    • Assessment and documentation of education and discharge needs throughout hospitalization.
    • Patient reassessment with documentation of observations and response to care.
  • Establish, coordinate and evaluate a plan of care.
    • Identify short‑ and long‑term goals based on patient needs.
    • Formulate interventions to achieve desired outcomes.
    • Incorporate disease‑specific evidence‑based practice.
  • Provide and document nursing interventions.
    • Collaborate with team members for daily plan of care.
    • Communicate patient care and handoff reports.
    • Administer and document medications accurately.
    • Monitor and document IV fluids and parenteral nutrition.
    • Complete referrals, request consultations, and provide patient/family education.
    • Clarify and acknowledge physician orders.
    • Assist physicians with procedures or delegate to Care Partner.
    • Document “readback” for all telephone/verbal orders; take orders only in emergencies.
    • Recognize changes in condition and take appropriate actions.
    • Involve family/guardian in care and decision‑making.
    • Identify risks and implement infection control practices.
  • Document and communicate nursing care.
    • Ongoing evaluation of effectiveness of care.
    • Evaluate patient/family education.
    • Modify plan of care as indicated.
    • Report significant changes immediately.
    • Identify problems, gather data, suggest solutions, and work toward resolution.
    • Report variations from care per policy.
  • Specialized Care
    • Restraint Care
    • Initiate/evaluate alternatives prior to application.
    • Apply restraints consistently.
    • Monitor patient response at appropriate intervals.
    • Provide toileting, skin care, hydration, feeding, etc., on time.
    • Consult peers for alternatives and 1:1 observation.
    • Document restraint use and associated care.
  • Pain Management
    • Assess pain on admission and during assessments.
    • Incorporate cultural/spiritual beliefs into plan.
    • Implement preventive pain techniques.
    • Include patient/family in plan development.
    • Consider massage, repositioning, immobilization, music therapy.
  • Abuse Assessment
    • Recognize abuse criteria during assessments.
    • Report signs to appropriate authorities.
    • Take appropriate action to support patient safety.
  • Accountability for Professional Practice
    • Adhere to all quality and performance standards, policies, and protocols.
    • Participate in learning experiences to increase competence.
    • Maintain currency in policies, procedures, and unit information.
    • Respond positively to change and support peers.
    • Adapt to unplanned workload and job demands.
  • Relationship with Coworkers
    • Orient and precept new employees as required.
    • Collaborate with the team and serve as a resource person.
    • Delegate care and activities appropriately.
    • Redirect staff when the plan of care changes.
    • Communicate the plan of care precisely.
    • Supervise and teach students/new staff nurses.
  • Safety and Infection Control
    • Practice standard precautions and dispose of hazardous waste per guidelines.
    • Maintain a safe, clean, and therapeutic environment.
    • Follow hand hygiene and safe patient handling techniques.
    • Report risk management concerns via eSRM.
    • Complete all annual mandatory requirements (license, certifications, trainings).
    • Report variations from care per policy.
