The Registered Nurse, in Home Health
, provides professional nursing services in accordance with physicians’ orders or plan of care. The nurse will be responsible for completing Start of Care (SOC), Resumption of Care (ROC), Recertification, Skilled Nursing Visits (SNV), OASIS Discharge, and Discharge Summaries. This position is not included in the on-call rotation and does not have a designated number of scheduled hours. The nurse will not function as the primary case manager for patients they admit but will be accountable for accurate and timely completion of all clinical documentation and for effectively communicating pertinent patient information to the assigned case manager and interdisciplinary therapy team.

The RN Admissions Nurse provides professional nursing services in accordance with physicians’ orders or plan of care. The RN Admissions Nurse is responsible for performing the initial visit with the patient/resident to review the FV at Home admission booklet, obtain signatures on consent forms, and establish the plan of care in coordination with the patient/resident, ordering physician, and other disciplines participating in the care of the patient/resident.

• Licensed as a registered nurse in the state in which employed.
• Current Basic Life Support (BLS) certification or ability to obtain prior to direct patient contact.
• Valid driver’s license and proof of insurance required.
