Overview

Recognizing its excellence in nursing care, Barnes-Jewish Hospital was the first adult hospital in Missouri to be certified as a Magnet Hospital by the American Nurses Credentialing Center.

The CM Services reports through the division of Patient Care Services lead by the Chief Nursing Executive, Vice President of Patient Care Services. The CM Service is comprised of three departments:
Case Management, Utilization Management, and Social Work. Through the Triad Model of Care Delivery, the objectives of the CM Services are for all patients to have access to healthcare through the BJH continuum and appropriate, safe discharge plans. The SW department is open 24 hours/day while the CM department is open 20 hours/day. Both departments provide coverage throughout the hospital including the Emergency Department, Outpatient Clinics, Registration sites, and Health Information Management.

The Service employs registered nurses, social workers, a licensed practical nurse, a customer service representative, and administrative assistants.

Onsite position with benefits eligible and an excellent team culture. Schedule typically Monday - Friday; days; 8am-4:30pm with flexibility as needed. On-call requirement with weekend rotation every 6 weeks, including after hours.

Responsibilities
  • Supervises part of the RN Case Coordination team at Barnes Jewish Hospital (30-35 employees).
  • Provides ongoing support to the care coordination program and staff by mentoring, oversight, training, and ongoing staff development.
  • Works with departmental management to ensure designated aspects of the program are functioning in the highest quality.
  • Manages operations of unit services and integration with other departments; hires, manages, and evaluates staff and services to build an effective work unit.
  • May participate in the development of departmental staffing, revenue and/or expense budgets and have direct responsibility for adhering to those goals.
Minimum Requirements
  • Education:

    Bachelor's Degree;
    Nursing
  • Experience:

    2-5 years
  • Supervisor

    Experience:

    No Experience
  • Licenses &

    Certifications:

    RN
Preferred Requirements
  • Ideal candidate will have: MSN
  • 2-5 years of RN experience
  • At least 2 years of leadership experience
  • Acute Care Case Management Experience
