Our innovative care model is integrated into the communities of our patients and focuses on the quality of care rather than the volume of services.

• Salary Range: $105,000 - $135,000 (Experienced providers with bonuses can earn $150,000 - $160,000)
• Annual Clinical Performance Bonus:
  • Less than 5 years of experience: $15,000
  • 5+ years of experience: $25,000

• Focus: Post-discharge visits, including:
  • Medication reconciliation
  • Social service follow-ups
  • General follow-up care and care coordination
• Caseload: 6-7 patients per day
• Support: Medical Director available for guidance

• Master of Science in Nursing, Doctor of Nursing Practice, or Nurse Practitioner Certificate Program graduate
• National certification in at least one of the following specialties:
  • Family Nurse Practitioner
  • Adult-Gerontology Primary Care Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontological Nurse Practitioner
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US work authorization
