RN Care Manager II-IPCM (Exempt)

Mercy is looking for an RN Care Manager II-IPCM (Exempt) to join our interdisciplinary team. The Care Manager will assess, plan, advocate, and coordinate care from admission to discharge, ensuring a safe transition post hospitalization while aligning with Mercy’s mission, values, and service standards.

• Graduate of an accredited School of Nursing – required.
• Bachelor’s Degree of Science in Nursing – minimum required.
• Experience:
  
  2‑3 years in an acute care hospital setting (preferred). Care Management or Utilization Management experience (preferred).
• Licensure:
  Current license in the state of employment – required.
• Certifications:
  
  BLS (CPR) at hire date required, or within 90 days of hire;
  Certification in Case Management (preferred).
