Hospice Licensed Practical Nurse (LPN) – PRN

Pay: $25.00 – $30.00 per hour, depending on experience.

• Provides technical care to patients, receives assignments and reports, and administers prescribed medication under the supervision of a registered nurse.
• Observes, records, and reports all observations of the patient’s mental and physical care.
• Performs special procedures or treatments supervised by an RN.
• Notifies the Patient Care Coordinator and/or physician immediately of any changes in the patient’s condition and documents changes in the clinical record.
• Meets periodically with the Patient Care Coordinator to discuss nursing care, policies, and future planning.
• Acts as a link to help the agency comply with regulatory requirements.
• Participates as an interdisciplinary team (IDT) member and formulates the care plan with other IDT members.
• Assists home health aides and the RN or physician with specialized procedures when requested.
• Utilizes required automated devices to document in the electronic medical record accurately.
• Participates on call according to a rotating schedule.

• Licensed in the state of operation.
• At least one year of nursing experience and complete knowledge of technical skills required in nursing procedures.
• Understanding of hospice care concepts and role as a member of an interdisciplinary care team.
