Position: RN Home Healthcare - Full-Time
Location: St. LouisWe are looking for a Registered Nurse to join our growing team. You will be among a group of professionals that are competent, knowledgeable, and compassionate about patients and the medical profession. We are a diverse company that employs inclusion.
The RN is responsible for educating patients and their families on prevention, illnesses, and medications, as well as providing wound care, injections, and other nursing functions.
The ideal candidate will be a responsible and well-trained professional able to give the best nursing care with little supervision. An upbeat, compassionate nurse with patience, you will be able to follow health and safety guidelines faithfully and consistently. The goal is to promote patients’ well-being by providing high-quality nursing care.
Responsibilities:
Perform the initial home care patient visit and re-evaluate the patient’s needs and progress on a regular basis.Initiate the plan of care under doctor’s orders.Perform admission, transfer, re-certification, resumption of care, and discharge Oasis for the home care patient.Monitor reactions and patient progress.Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.Educate patients and caregivers on techniques for in-home health care.Coordinate patient services.Supervise LPNs and HHAs.Notify the physician and other personnel (Clinical Manager, Therapist, and Case Manager) of changes in the patient’s condition.Perform skill outline in the agency’s approved policy and procedure manual.Discharge the patient from skilled nursing services when the discharge criteria have been met.Case conference clinicians providing care to ensure coordination of care.Update clinical records according to policy and procedures.Update knowledge and skills by attending in-service programs, continuing education programs, seminars, and self-study programs annually.Provide onsite supervision of LPN/HHA.Adhere to Medicaid and Medicare regulations.Skills:
Proven experience as a registered nurse.Excellent knowledge of nursing care methods and procedures.Excellent knowledge of home healthcare.In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.) and willingness to follow them at all times.A team player with excellent communication and interpersonal skills.Responsible and compassionate.Outstanding organizational and multi-tasking skills.Patient with excellent problem-solving skills.Valid nursing license in Missouri.#J-18808-Ljbffr
