RN (Registered Nurse) Hospice Case Manager

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. They work with the interdisciplinary team to develop and implement patient/family centered plans of care, facilitating goals of care discussions and providing guidance with healthcare decisions to promote quality of life.

Full Time & PRN Availability. St. Louis, MO and surrounding areas.

We are Medicare certified and state licensed.

• Provide education to patients/family members regarding the process, plan of care, and offer guidance with healthcare decisions
• Consult with the attending physician and Hospice Medical Director concerning changes in patient’s condition requiring order changes and/or changes to plan of care
• Coordinate hospice care by working with other team members as well as with the patient/family members to ensure needs are met
• Conduct initial as well as on‑going comprehensive assessments
• Perform evaluations for hospice care along with informational visits

• Missouri RN license
• Hospice experience preferred but not required
• Valid driver's license and auto liability insurance
