The professional registered nurse Outpatient RN CAP 2 may be an entry‑level designation into the Clinical Advancement Program (CAP) for new employees who meet the RN CAP 2 criteria or have advanced from RN CAP 1. The position is accountable for providing and coordinating clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. Duties include delegation of tasks to direct patient care staff such as RNs, LVN/LPNs, and Patient Care Technicians.
As a member of the End Stage Kidney Disease (ESKD) health care team, the RN participates in decision‑making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and facility operations.
- Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor, or in collaboration with other Registered Nurses.
- Collects and analyzes patient data pre–, during–, and post hemodialysis treatment, documents in the patient medical record, and makes appropriate adjustments to treatment plans.
- Assesses, collaborates, and documents patient/family learning needs to provide education.
- Directs daily safe and effective hemodialysis therapy delivery.
- Administers medications as prescribed, documenting justification and effectiveness.
- Initiates or assists with emergency response measures.
- Serves as a resource, leader, coach, mentor, and role model for new and incumbent employees.
- Ensures correct laboratory collection, processing, and shipping procedures; reschedules missed or insufficient collections.
- Identifies expected outcomes, updates the nursing assessment and plan of care, and collaborates with the interdisciplinary team.
- Ensures patient awareness of transplant and modality options.
- Participates in education and quality improvement projects at facility and area level.
- Serves as a preceptor to new employees when required.
- Completes CAP requirements to maintain or advance.
- Performs other duties as assigned by Supervisor.
- Direct patient care involving heavy lifting, moving patients, and ambulation.
- Frequent, prolonged periods of standing; ability to bend over.
- May need to move machines and equipment up to 200 lbs.
- May be exposed to infectious and contagious diseases/materials.
- Desk and computer work, interaction with patients and physicians.
- May require travel to training sites or other facilities.
- Provides oversight of Patient Care Technicians, LVN/LPNs, and RNs as Team Leader or designated Nurse in Charge pending completion of FKC training and minimum 3 months experience in chronic/acute hemodialysis as RN.
- Graduate of an accredited School of Nursing.
- Current state licensure (RN).
- Current CPR BLS Certification.
- Entry level for RNs with minimum 2 years of Nephrology Nursing experience.
