Job Responsibilities and Requirements

Essential Functions:

  • Comprehensive Nursing Assessment
    • Performs age‑appropriate admission or transfer assessment. Obtains input from family/guardian when appropriate.
    • Accurately documents findings.
    • Assesses post‑op/post‑invasive procedure patients.
    • Documents education and discharge needs on admission and throughout hospitalization.
    • Provides reassessment based on plan of care, changes in condition, status, diagnosis, response to care, procedures, etc.
  • Plan of Care Development
    • Establishes, coordinates, and evaluates a plan based on assessment data, patient diagnosis, lab data, tests, procedures, orders, protocols, and standards.
    • Identifies short and long‑term goals based on patient needs.
    • Formulates nursing interventions to achieve desired outcome.
    • Incorporates disease‑specific evidence‑based practice into care plan and documentation.
  • Interventions and Documentation
    • Provides and documents interventions based on plan of care.
    • Collaborates with health team for daily plan coordination.
    • Communicates patient care and handoff reports accurately.
    • Administers medications and documents per policies.
    • Maintains IV fluids and parenteral nutrition per procedures.
    • Completes referrals, requests consultations, provides education and discharge planning per guidelines.
    • Clarifies and implements physician orders timely and accurately.
    • Assists physicians with procedures or delegates to Care Partner as appropriate.
    • Documents readback for telephone/verbal orders and takes orders only in emergencies.
    • Recognizes changes in condition and acts appropriately; involves family/guardian when providing care.
    • Recognizes risks and applies infection‑control practices daily.
  • Evaluation of Care
    • Documents and communicates changes in patient condition.
    • Evaluates effectiveness of care and education, and modifies plan as needed.
    • Reports significant changes to physicians immediately.
    • Identifies problems, gathers data, suggests solutions, and works toward resolution.
    • Reports variations from care/treatment as per policy.
  • Specialized Care
    • Restraint Care:
      Initiates/evaluates alternatives, applies restraints per procedure, monitors response, provides required care, consults peers, documents use.
    • Pain Management:
      Assesses pain, incorporates cultural beliefs, implements techniques, includes family in plan, considers alternative methods.
    • Abuse Assessment:
      Recognizes criteria, reports signs to appropriate authorities, takes actions to support patient safety.
  • Professional Practice Standards
    • Adheres to quality and performance standards, policies, and protocols.
    • Participates in learning experiences; maintains competence.
    • Keeps current with hospital/unit information, communications, policies, and procedures.
    • Participates in committees, governance, shared teams, and unit-based practice councils.
    • Demonstrates adaptability to change.
    • Assists in change management; supports peers.
  • Teamwork and Leadership
    • Provides orientation and precepting for new staff.
    • Maintains collaborative team approach.
    • Serves as resource to co‑workers, patients, families, and staff.
    • Delegates and assumes care responsibilities appropriately.
    • Redirects clinical staff when plan changes.
    • Communicates care plans precisely and professionally.
    • Supervises and teaches students/new staff.
  • Safety / Infection Control
    • Practices standard precautions and disposes of hazards per guidelines.
    • Maintains safe, clean, therapeutic environment; follows hand hygiene, safe handling techniques, and dress code.
    • Reports risk management concerns via eSRM.
    • Completes all mandatory requirements: license/certification, BLS, HI, TB screening, influenza vaccination, ethics, safety, information security, abuse & neglect, blood‑borne pathogens, education, competencies, service‑specific training.
  • Service Excellence
    • Provides courteous, respectful service; meets patient emotional, physical, spiritual needs.
    • Coordinates teamwork to answer call lights promptly.
    • Apologizes for delays; communicates effectively and confidentially.
    • Provides accurate handoff reports and shares knowledge to facilitate workflow.
