Acute Care Medicine nurses manage patients with a full range of clinical issues—including significant blood dyscrasias, advanced endocrine pathology, heart and renal failure, GI conditions, infectious diseases, asthma, cystic fibrosis, and many other conditions. Because of patient acuity, nurses receive advanced training to operate at the highest level of functioning. Each unit is led by a clinical manager and lead charge nurse, supported by an advanced practice nurse and a multidisciplinary team that includes case managers, social workers, physical therapists, dietitians, and pharmacists.
Responsibilities- Promotes patient and family‑centered care in a healing environment. Educates patients and families on illness management, post‑treatment home care needs, and medication administration.
- Participates in activities that promote patient safety, quality, and regulatory compliance; participates in professional development.
- Develops, implements, and documents individual plans of care with defined goals in collaboration with the interprofessional team and the patient, family, or caregiver, ensuring communication across the continuum of care.
- Uses critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to standards of care.
- Assesses patient preferences, values, emotions, spirituality, culture, and barriers to involvement in care.
- Works in a constant state of alertness and in a safe manner, as this is a safety‑sensitive position.
