Location: St. Louis

Position Summary

We are seeking an experienced Medical Records Coordinator to manage and maintain resident medical records from pre-admission through post-discharge. This role ensures accuracy, confidentiality, and full compliance with state and federal regulations, professional standards, and company policies while supporting billing accuracy and quality outcomes.

Responsibilities
  • Maintain accurate, complete, and confidential resident medical records in compliance with HIPAA, state, and federal regulations
  • Manage medical records from admission through discharge, including chart creation and maintenance
  • Perform ICD diagnostic coding to support accurate billing and reimbursement
  • Conduct monthly audits (admissions, discharges, QA reviews)
  • Track incomplete records and coordinate corrections with nursing staff
  • Prepare reports for QAPI and Triple Check meetings
  • Maintain admission/discharge registries and census reports
  • Process authorized release of information and legal correspondence
  • Abstract records for insurance companies and third-party payers
  • Ensure protection of medical records from loss, destruction, or confidentiality breaches
  • Maintain Point Click Care  documentation and tracking systems
  • Assist with surveys and regulatory reviews
Qualifications
  • Minimum 2 years of medical records experience in a skilled nursing facility or healthcare setting
  • Knowledge of medical terminology, ICD coding, and long-term care regulations
  • Experience with Point Click Care  (preferred)
  • Strong organizational and communication skills
  • Ability to work collaboratively with residents, families, staff, providers, and regulatory agencies
  • CNA or CMT certification required
  • RHIA or RHIT credential preferred
