Position Summary
We are seeking an experienced Medical Records Coordinator to manage and maintain resident medical records from pre-admission through post-discharge. This role ensures accuracy, confidentiality, and full compliance with state and federal regulations, professional standards, and company policies while supporting billing accuracy and quality outcomes.
Responsibilities- Maintain accurate, complete, and confidential resident medical records in compliance with HIPAA, state, and federal regulations
- Manage medical records from admission through discharge, including chart creation and maintenance
- Perform ICD diagnostic coding to support accurate billing and reimbursement
- Conduct monthly audits (admissions, discharges, QA reviews)
- Track incomplete records and coordinate corrections with nursing staff
- Prepare reports for QAPI and Triple Check meetings
- Maintain admission/discharge registries and census reports
- Process authorized release of information and legal correspondence
- Abstract records for insurance companies and third-party payers
- Ensure protection of medical records from loss, destruction, or confidentiality breaches
- Maintain Point Click Care documentation and tracking systems
- Assist with surveys and regulatory reviews
- Minimum 2 years of medical records experience in a skilled nursing facility or healthcare setting
- Knowledge of medical terminology, ICD coding, and long-term care regulations
- Experience with Point Click Care (preferred)
- Strong organizational and communication skills
- Ability to work collaboratively with residents, families, staff, providers, and regulatory agencies
- CNA or CMT certification required
- RHIA or RHIT credential preferred
