Concordance’s reentry program is designed to dramatically reduce recidivism by helping individuals with justice involvement lead meaningful, abundant, and joyful lives. The Post-Release Therapist supports participants on their healing journey. Working alongside a comprehensive care team, you will employ our healing-first approach and help participants create and achieve the goals in their treatment plan.
Key Responsibilities- Conduct impactful weekly 1:1 and group sessions using Cognitive Behavioral Therapy and Motivational Interviewing to support participants in achieving their goals.
- Provide frequent, supportive communications to participants throughout post-release phases.
- Update participants’ objectives and goals in their treatment plans.
- Use assessments to confirm treatment plan goals and progress.
- Uphold the quality and fidelity of Concordance Re-entry Model curriculum when delivering 1:1 and group sessions.
- Prepare for sessions in advance to maximize effectiveness of interactions.
- Evaluate participant progress and support needed, using assessments and other evaluations as needed.
- Document sessions (for individuals and groups), including noteworthy participant interactions in the electronic record system.
- Work collaboratively with participants’ care teams, providing input on progress and flagging holistic participant needs.
- Participate in internal meetings (Behavioral Health and Wellness team meetings, All Team meetings, 1:1 supervisor meetings, and training sessions). Occasionally help set up for Center events or move classroom around, including but not limited to carrying and setting up tables and chairs.
- Passion for our mission.
- Ability to resolve conflicts in crisis situations, redirect and set boundaries, and work with a diverse team.
- Collaborative attitude and excitement about working in a multi-disciplinary environment.
- Flexibility and adaptability to balance priorities.
- Effective communication skills with the broader care team.
- Skill in documenting interactions in case notes and flagging participant needs.
- Commitment to professionalism, integrity, and compassion in all interactions.
- Master’s degree in counseling, social work, behavioral health, or related field.
- Independent clinical license.
- 3-5 years of experience working in mental health and substance use disorders, with equal focus on mental health and substance use treatment.
- Familiarity with and endorsement of 12-step program, and equal experience with traditional Cognitive Behavioral Therapy interventions.
- CPR / Adult First Aid certification preferred.
